ZyCare has chosen to introduce its expert system approach by addressing a very specific opportunity to facilitate "Physician-Supervised Patient Self-Management"™ of oral anticoagulation therapy with warfarin (Coumadin®). While warfarin therapy is relatively inexpensive and well tolerated, its potency as an anticoagulant depends up what foods and medications a patient is using. Therefore, it is not easy for the physician to get the dosage just right. If a patient is too anticoagulated he can suffer from dangerous bleeding, while too little anticoagulation can leave the patient at risk for blood clots and strokes. Consequently, most patients on warfarin must visit their doctor once or twice a month to have their blood tested and their warfarin dosages adjusted. Frequent communication between visits is often required. This is inconvenient for the patients, and very costly for the managing physicians.
To solve this problem, ZyCare has created the CoagCare system. CoagCare is an Internet-enabled expert system. Both patients and healthcare providers can access the system through a browser from any location. All data are encrypted and secure. Because not all patients are candidates for patient self-testing (PST), CoagCare offers a complete solution to the management of patients receiving point-of-care management as well as those receiving supervised PST. In the clinic, caregivers can access the point-of-care module to enter patient data, adjust warfarin dosages according to a pre-defined algorithm and schedule new appointments. In addition, health care providers using the CoagCare system can cost-effectively supervise large numbers of PST patients remotely.
With PST, CoagCare routinely queries patients for symptoms and clotting time values, as measured by a home INR monitor, and provides instant feedback to the patient regarding changes in warfarin doses or the need to seek professional attention. CoagCare uses this data to prioritize patients for physician review, and serves as a conduit for physicians to communicate personal instructions to the patient in a structured fashion. Thus, CoagCare provides patients with the convenience and safety of frequent monitoring at home, while the expert system functions of CoagCare enable physicians to supervise PST quickly and effectively using the automated oversight and clinical decision making features of the system. The automation provided by the CoagCare system has been shown in our clinical trials to reduce the workload of the healthcare provider while improving outcomes. CoagCare maximizes the use of professional time by screening all incoming patient data, enabling healthcare professionals to focus their attention on patients with significant problems. This will help reduce medical errors and help insure a uniform standard of care. Data for point-of-care patients as well those of PST patients can be accessed from the same interface. This capability is completely unique to CoagCare.
The Company plans to develop future applications of its technology for other chronic diseases such as congestive heart failure, asthma, and hypertension.
