ZyCare has developed the CoagCare system to facilitate supervised patient self-management of warfarin therapy. CoagCare is a browser-based "Direct-to-Patient Expert System"™ that will structure the management of patients using home self INR testing devices. The patient uses the Internet to connect to the CoagCare web page and enter the results of each home test. CoagCare prompts to the patient to answer questions about the presence of thromboembolic or hemorrhagic symptoms, changes in medications or major dietary or lifestyle changes. Software in the CoagCare server analyzes the results and uses a commonly accepted dosing algorithm to provide appropriate adjustments in warfarin dose. All data on the CoagCare server are continuously accessible to the patient's healthcare provider via the Internet. The CoagCare software flags patients with markedly abnormal results or recurrent problems for physician intervention, allowing the physician to focus on patients in need of attention. CoagCare also automatically changes the testing schedule based on PT data. Indications of problems increase the frequency of scheduled INR testing, while stable INR values in the absence of other problems decrease the frequency of testing. ZyCare believes that CoagCare enables healthcare providers to safely supervise the management of a larger number and wider range of patients, making effective anticoagulation therapy both more convenient for the patient and the physician and more affordable for the health care system.
Since CoagCare is web-based, patients can be in touch with their physicians from any location around the world, while the expert system features of the system dramatically reduce the burden of management for physicians.
